Request for Exclusion Login

If you want to be excluded from the IMWL and IWPCA class action, you must complete and sign an Exclusion Form. If you properly request to be excluded before the deadline, you will not be bound by the result of this lawsuit.

To exclude yourself, you must submit an Exclusion Request either online or by mail no later than January 30, 2026.

To start your online Exclusion Form you must login with your Notice ID and PIN. You can find your ID & PIN printed on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact Stephan Zouras, LLC at www.stephanzouras.com.
